The Plague of Justinian: The First Pandemic

Eight centuries before the Black Death devastated medieval Europe, an earlier pandemic struck the Mediterranean world with comparable ferocity. The Plague of Justinian, which erupted in 541 CE and recurred in waves for nearly two centuries, was the first recorded pandemic of bubonic plague. It struck the Byzantine Empire at the height of its power, killed millions, and reshaped the political and demographic landscape of the late antique world. Its study provides essential context for understanding the Black Death, as both pandemics were caused by the same bacterium, Yersinia pestis, and both reveal the devastating potential of plague when it encounters susceptible human populations.

The Outbreak of 541

The plague first appeared in the Byzantine Empire in the summer of 541, during the reign of Emperor Justinian I (r. 527–565). Justinian was one of the most ambitious rulers of late antiquity, engaged in a vast programme of territorial reconquest, legal codification, and architectural patronage. His armies had recently recovered North Africa from the Vandals and were campaigning to retake Italy from the Ostrogoths. The plague struck at a moment when the empire’s resources were stretched to their limits.

The disease emerged in Egypt, the empire’s breadbasket and most populous province. The historian Procopius of Caesarea, who witnessed the plague in Constantinople, recorded that it began at Pelusium, a port city at the eastern mouth of the Nile, and spread rapidly to Alexandria and the rest of Egypt. From Egypt, it moved by ship to Palestine, Syria, and Asia Minor, and by the spring of 542 it had reached Constantinople, the imperial capital.

Procopius’s account of the plague in Constantinople, written in his History of the Wars, is one of the most detailed and reliable primary sources for any ancient epidemic. He described the characteristic symptoms — fever, swelling of the lymph nodes (buboes), delirium, and death — with clinical precision. He noted that the disease struck without warning, killed within days, and spared no age, sex, or social class. He estimated that 5,000 to 10,000 people were dying daily in Constantinople at the epidemic’s peak, and that the total death toll in the city was enormous.

Emperor Justinian himself contracted the disease but survived. The epidemic’s impact on his military campaigns was significant: the reconquest of Italy, which had been proceeding successfully, was hampered by the loss of soldiers and the disruption of supply lines. The empire’s fiscal base contracted as tax revenues fell, and the demographic losses weakened the state’s capacity to defend its frontiers.

The Spread Across the Mediterranean

From Constantinople, the plague spread throughout the Mediterranean world over the following years. It reached Italy in 543, where it struck the cities of the peninsula — Rome, Ravenna, Naples, Milan — during the final stages of the Gothic War. The Ostrogothic kingdom, already weakened by decades of warfare, was further devastated by the epidemic.

The plague reached Gaul (modern France) in 543, carried by ships from Italy. The bishop Gregory of Tours, writing in the late sixth century, described the plague’s impact on the Frankish kingdom in his History of the Franks. He recorded outbreaks in Arles, Lyon, and other cities, and noted the terror and social disruption that accompanied the disease.

The plague also spread eastward, reaching Persia and the Sasanian Empire. The Persian historian al-Tabari, writing in the ninth century but drawing on earlier sources, described plague in the Sasanian domains. The epidemic’s impact on the Persian Empire is less well documented than its impact on Byzantium, but it was likely significant.

The British Isles were affected in the 540s, though the evidence is sparse. The Welsh monk Gildas, writing in the mid-sixth century, may have alluded to the plague in his De Excidio et Conquestu Britanniae, and Irish annals record a “great mortality” in the 540s that may represent plague.

Mortality and Demographic Impact

The total death toll of the Plague of Justinian is one of the most debated questions in late antique history. Contemporary sources provide figures that range from the plausible to the fantastical. Procopius’s estimate of 5,000 to 10,000 daily deaths in Constantinople, if sustained over the several months of the epidemic’s peak, would imply a total death toll in the city of several hundred thousand — a figure consistent with the city’s estimated population of 500,000 to 750,000.

Modern estimates of the pandemic’s total mortality vary widely. Some scholars, following the historian Josiah Russell, have estimated that the plague killed approximately twenty-five million people across the Mediterranean world — roughly a quarter of the region’s population. Others, including the historian Kyle Harper, have argued for higher figures, suggesting that the plague and its subsequent waves may have killed as many as fifty million people over the course of two centuries.

The demographic impact was uneven. Cities, with their dense populations and maritime connections, suffered more heavily than rural areas. The Mediterranean ports, which were the primary nodes of plague transmission, were struck repeatedly, while isolated rural communities sometimes escaped infection entirely.

Recurring Waves

The Plague of Justinian was not a single event but a pandemic that recurred in waves over nearly two centuries. Major outbreaks are recorded in 558–561, 573–574, 590, 599–600, 608–609, 618–619, 627–628, 638–639, 670–671, 687–689, and 746–747. Each wave killed a significant proportion of the population and prevented the demographic recovery that might otherwise have occurred between outbreaks.

The cumulative effect of these repeated epidemics was devastating. The population of the Byzantine Empire, which had been approximately twenty-six million at the beginning of Justinian’s reign, may have fallen to seventeen million or fewer by the end of the sixth century. The empire’s fiscal and military capacity was correspondingly reduced, and its ability to resist the Arab conquests of the seventh century was impaired.

The Plague and the Arab Conquests

One of the most significant historical consequences of the Plague of Justinian was its impact on the balance of power between the Byzantine and Sasanian empires and the rising Arab caliphate. Both the Byzantine and Sasanian empires were weakened by the plague’s demographic and economic effects, and when Arab armies burst out of the Arabian Peninsula in the 630s, they encountered states that were demographically depleted, fiscally strained, and militarily overstretched.

The plague’s impact on the Byzantine Empire was particularly significant. The loss of population reduced the empire’s tax base and its ability to recruit soldiers. The disruption of trade and agriculture weakened the economy. The repeated epidemics of the late sixth and early seventh centuries prevented demographic recovery and left the empire vulnerable to the Arab advance.

The Arab conquests of Syria, Egypt, and North Africa in the 630s and 640s were facilitated by the weakened state of the Byzantine provinces. Whether the plague was a decisive factor in the Arab success is debated — the conquests had multiple causes, including political instability, religious divisions, and the military effectiveness of the Arab armies — but the demographic and economic impact of the plague undoubtedly played a role.

Molecular Evidence

The identification of the Plague of Justinian as bubonic plague was confirmed in 2013, when a team led by Michaela Harbeck recovered Yersinia pestis DNA from the teeth of individuals buried in a sixth-century cemetery at Aschheim, near Munich; a 2016 study led by Michal Feldman later reconstructed a high-coverage genome from the same site. The DNA analysis showed that the Justinianic strain of Y. pestis was genetically distinct from the strain that caused the Black Death, representing an earlier branch of the bacterium’s evolutionary tree.

Subsequent studies have recovered additional Justinianic plague genomes from cemeteries across Europe, providing a more detailed picture of the bacterium’s diversity during the first pandemic. These studies have shown that the Justinianic strain was closely related to modern Y. pestis strains found in Central Asia, supporting the hypothesis that the pandemic originated in the same rodent reservoirs that would later produce the Black Death.

The molecular evidence also reveals that the Justinianic strain went extinct — it has no known modern descendants. The Black Death strain, by contrast, is the ancestor of nearly all modern plague-causing bacteria. This suggests that the two pandemics represent independent emergences of Y. pestis from its natural reservoir, rather than a continuous chain of transmission.

Comparisons with the Black Death

The parallels between the Plague of Justinian and the Black Death are striking. Both pandemics were caused by Yersinia pestis. Both emerged from the rodent reservoirs of Central Asia. Both spread along trade routes that connected distant regions. Both struck urban populations with particular severity. Both recurred in waves that prevented demographic recovery. And both had profound political, economic, and cultural consequences.

The differences are also instructive. The Plague of Justinian occurred in a world with less developed commercial networks and lower population densities than the fourteenth century, which may have limited the speed and extent of transmission. The medical and institutional responses to the two pandemics reflected the very different intellectual and cultural contexts of late antiquity and the high Middle Ages. And the long-term consequences diverged: while the Black Death contributed to the transformation of medieval European society, the Plague of Justinian contributed to the decline of the late antique Mediterranean world and the rise of Islamic civilisation.
